Connection not transparent and no plug-to address in session-acl

Description

If the client connection is non-transparent, i.e., its destination address is the address which the proxy listens on, item PLUG-TO must exist in the corresponding ACL. Otherwise the proxy does not know to which server it should connect and the session is terminated.
